We are seeking a compassionate and driven Physician to build and manage a dedicated panel of patients across independent and assisted living facilities. \n\nEssential Duties & Responsibilities:\nPatient Panel Development : Build and maintain strong patient relationships by developing a dedicated panel and providing continuity of care.
Direct Patient Care : Conduct assessments, manage chronic conditions, and deliver preventive services as the primary physician for residents.
Routine Rounds : Perform regular visits at assigned facilities, addressing both ongoing and acute health needs.
Care Coordination : Partner with facility staff, families, and interdisciplinary teams to create personalized, holistic care plans.
Documentation & Compliance : Ensure accurate, timely EMR documentation while adhering to all regulatory standards.
Patient & Family Education : Provide education on preventive health, chronic condition management, and wellness strategies.
Quality Improvement : Participate in initiatives that drive better outcomes and elevate the standard of care.

This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required.
