Job Description & Requirements Job Summary:
\nProvides excellent patient care by assisting in collaboration, development, implementation, revision and reporting of the case management program.
\nActs as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items.\n\n \n\nEducation and Work Experience:
\nBachelor's Degree in Nursing (BSN): Preferred\n\n \n\nLicenses/Certifications:
\nRegistered Nurse (RN) licensure in the state of practice: Required
\nCase management certification: Preferred
\nBasic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required
