Community Health Worker

Qualification and Education Requirements 

• High School Diploma or equivalent
• Completed TDH approved CHW Certification course
• Current Driver's License & Reliable Transportation
• Excellent observation, communication and relationship building skills
• Proven experience in healthcare or community outreach role
• Ability to work independently and collaboratively within a team
• Experience with EMR software is a plus

Essential Duties

• Assists with recruitment and enrolment or participants in home setting
• Provides emotional support to patient and their families, fostering open communication and support
• Assists staff in identification and connection to resources for patients and families
• Develops relationships with community organizations and facilities to secure resources and helps to complete appropriate applications
• Assists in coordinating home health services, medical equipment and things related to medication management
• Maintains accurate and up to date documentation of up-to-date documentation of patient interactions and interventions
• Ensure compliance with all relevant healthcare regulations and standard
• Performs other duties as required.
